Before Your First Visit

We recommend you fill out our new patient forms ahead of time to make your check-in process smoother.
- 🏥 Medical History:
If you require any serious health accommodations or pre-medication, please let us know.
- 💳 Insurance Info:
Please bring your insurance card and ID. We'll verify your coverage before your visit.
- ⏰ Arrive Early:
Plan to arrive 10-15 minutes early to check in for your visit.
Your First Appointment
Pediatric Dental Exam or Orthodontic Evaluation:For a pediatric dental visit, we'll examine your child's teeth and take any necessary radiographs. For an orthodontic consultation, we'll evaluate the patient's teeth, bite, jaws, and orthodontic needs.
Consultation:We'll discuss findings and answer all your questions.
Treatment Plan:We'll create a personalized plan with clear options and costs.
Pediatric Cleaning (if needed):For pediatric dental patients, we may perform a professional cleaning during the visit. Routine adult dental cleanings are not provided at this time.
